Endovascular aneurysm repair (or endovascular aortic repair) (EVAR) is a type of endovascular surgery used to treat pathology of the aorta, most commonly an abdominal aortic aneurysm (AAA). The procedure involves the placement of an expandable stent graft within the aorta to treat aortic disease without operating directly on the aorta. EVAR has now surpassed open aortic surgery as the most common technique for repair of AAA.

Aneurysm shape is important, as a saccular aneurysm may be more prone to rupture than a fusiform aneurysm. Endovascular aneurysm repair (EVAR) is the treatment of choice for abdominal aortic aneurysms (AAA). One of the post-operative complications encountered after EVAR is an endoleak. Endoleak is where blood flow remains to the excluded aneurysm sac.

AAA repair is indicated at a diameter of 5 to 5.5 cm when the risk of rupture outweighs the risk of perioperative death or complication. The term “endoleak” was first coined by White et al. [ 1] and is defined as leakage of blood into an excluded aneurysm sac after stent-graft placement. Endovascular aneurysm repair (EVAR), is a type of minimally-invasive endovascular surgery used to treat pathology of the aorta, most commonly an abdominal aortic aneurysm (AAA). When used to treat thoracic aortic disease, the procedure is then specifically termed TEVAR for "thoracic endovascular aortic/aneurysm repair." Abdominal aortic aneurysm is a significant cause of morbidity and mortality in the United States. Endovascular aneurysm repair (EVAR) is the preferred treatment modality. Surveillance imaging after EVAR detects potential complications. The most common complication is endoleak, which can predispose the aorta to rupture.

2009-10-01 Open repair of AAA involves lengthy surgery time, anesthesia, and substantial recovery time. Endovascular aneurysm repair (EVAR) provides a safer option for patients with advanced age and pulmonary, cardiac, and renal dysfunction. Endovascular abdominal aortic aneurysm repair (EVAR), first described in 1991, is an alternative to traditional open repair. A stent-graft consisting of a metallic stent framework covered with a synthetic fabric material is placed in the lumen of the aneurysm.

If elderly patients develop a 5·5 cm diameter abdominal aortic aneurysm, do they benefit most from open surgical repair, endovascular aneurysm repair (EVAR), or no intervention?
